2026 Term1 - Enrolment & Agreement

Bloom - Enrolment Process

 

To enrol your child in our “one day a week programme” please follow these steps:

 

  1. Fill out this enrolment and agreement form.

  2. Read out and agree to our Terms and Conditions.

  3. Wait for our confirmation by email.

  4. Proceed with term payment.

 

Next one day program available will take place in 2026 during Term 1, from Wednesday 4th February to 1st April. 

 

Cost per session for 1 child: $75 (full term commitment required of 9 sessions $675)

 

Payments are to be made by bank transfer:

Reference: indicate your child's surname and T# - ie. SmithT1

 

Full payment should be made prior to the start of the first session.

If enrolling more than one child, please complete the Enrolment Agreement Form again separately for each extra child.

A category (i) medicine is a non-prescription preparation that is not ingested, used for the ‘first aid’ treatment of minor injuries and provided by the service and kept in the first aid cabinet.

Which of the followig are you happy with: Required

Category (ii) medicines are prescription (such as antibiotics, eye/ear drops etc.) or non-prescription (such as paracetamol liquid, cough syrup etc.) medicine that is used for a specific period of time to treat a specific condition or symptom, provided by a parent for the use of that child only.
